Programar con un formato de Entrada de Datos en Php y Mysql en pagina existente de Wordpress

Dear Programer,

The project requires to add a Script using PHP and accesing a Database in Mysql to an existing Wordpress webpage.

The dashboard and initial access to Client data base page has been created and will require to enter more data information on a newly Script format using the similar PHP format as the first format as Client.

For faster elaboration we recomend using a Scriptcase program www.scriptcase.com or similar and implement the resulting database and PHP script into the Wordpress website created.

Attached you will find the existing image of the Wordpress Client data entering format created along with the exterior Dashboard.

Also attached is the next format of the second Data input format needed to be created for Purveyors, Listing format and Dashboard format Presentation.

Please be certain that the above mentioned formats require to be implemented in the same format as previous and exact diementions, colors etc as all for them to be similar.

Please analyze your expected time to implement the requested task and if approved complete such implementation of your work in our hosting account.

We will take much consideration on previously PHP and Mysql related work done by the perspective programer, and specially the formating of such work using css and/or html formating to make the data entry formating most appealing and similar to formats given.
