Necesito un formulario en Html + css

con seleccion y en base a esta seleccion muestre un resultado con una imagen y un texto y un formulario de contacto.

Ej:

titulo de seleccion 1

opcion 1 opcion 2

desplegable en cada opcion

desp. 1 desp. 2

Titulo de seleccion 2

opcion 1 opcion 2 opcion 3 opcion 4

y en base a la seleccion debe mostrar una informacion debajo y un formulario de contacto (nombre y apellido, correo, y algun otro dato necesario)
