I need to have a website designed so my users can login, fill a form and submit it. It will show congrats after submit or error if anything goes wrong. All *.html was provided, except for the error one. It has to be developed using MDBootstrap as they will mainly (but not only) fill using their cell phones.  It's CSS needs to be easily mantained as the form will grow from time to time.
It can use any color scheme. I like pastel colors, purpleish and blueish gradients. The greetings screen must have hints of green and error screen hits of red.

Project overview

This form is part of a bigger system, even tho it is being released as stand alone, so it doesnt need to follow the system color scheme. The form will feed a database that i'll access from within my system and finish assisting my customers. You dont need to worry about coding the backend, only frontend. The form will change from time to time so the css coding must be easily understanded and replicated. I can answer questions in english and portuguese.
