Estou buscando empresa ou pj que seja ou tenha desenvolvedor + ux para desenvolvimento de app.

O app seria um formulário, que também possa receber fotos e faça coleta de localização georreferenciada.
Deve ter:
- dashboard com relatório, que eu possa filtrar por mês, quem preencheu etc;
- endereço web que tenha acesso a preencher o formulário sem ter que baixar o app.

A UX deve ser simples e lúdica, já que também será utilizado por crianças.
