Buen día requiero hacer un formulario.  De hecho sería justo algo así:

Ese formulario que está en el link lo realice con un plugin: FormCraft (le anexe el codigo en html para que se aperturará la opción de tomar la foto y si funciona pero al enviar el formulario no me llega la foto.)
