Convert 4 Page Word Document to Adobe Form

I have a four page word document that i need to convert to a fillable PDF form to send out to customers to fill in the fields and then sign. I need all the fonts to match in the text and the fillable fields. I need all the fillable fields to align. Need the date fields to be formated the same and in this format Feb 28, 2023. Need set up for two parties to sign but other signature related fields need to be fillable ... Printed name, title, etc. Need company logo added. Need the link to work. Need a footer on each page with AOTMP Unversity with the registered mark after the P. Along with our domain name, email contact and main phone number.. We can add the signature fields and send for signature. We will do that from Adobe Sign. Other minimum design is fine as long as it matches our web site / logo colors. I need two versions of the final document. The first one is the fillable form as described and ready to send for signatures. The second document would be a PDF form with the fillable fields but those fields would not be accessable and their should be a diagonal watermark saying Review Copy. No signatures or open fields would be needed on the Review Copy.  And finally, i need the Total Amount Due to calculate automatically from what is entered into the five other dollar fields. There are no cents, just whole dollars. Happy to have one video call to discuss in advance if preferred. Communication via email is preferred.
