Actualmente se tiene un formulario desarrollado en wordpress version 5.6 en un plugin llamado multi step form, el cual reune información de datos tipo texto y numérico, se necesita que estos datos lleguen a correos específicos por ciudad con base en la ciudad donde se diligencian y en formato PDF con un campo de firma al final.

Son 4 ciudades: Bogotá, Medellín, Cali y Barranquilla.
