Hi can you do for me  form submission with multiple attachments file ( docx, doc, pdf, jpg,jpeg,png , ppt,, audio file, video file) using php mailer ... Once user fill in the form then user can upload multiple attachment files at the same time.. Me, as admin can received all user submission form via my G mail account... Then at the same time, user also get the notification of their submission...

Email received must look like diiferent style .. Must have different block / text message / look interactive and stylish

The submission form must have
Full name
Email  address
Phone number
Message

Include responsive footer with
Logo
About us
Contact us
