I need a custom form for an enrollment process.

The system would need to have the following:

1- Generate user ID to fill the form, email it’s the login.
2- We have 3 types of forms.
3- We have the form on a word DOC.


4- It must be on PHP5+, CSS, html5, responsive 100% or something very scalable, Linux server, Zend Framework, zend guard.
5- We to know the status of the form status. If the user completes the minimal requirements.


6- UIX very user friendly
7- We need to print the form data in PDF format, on specific format
Something similar to the screenshot.

We need a freelancer or agency.

Ideally spanish spoken developers.
