I need a simple form to collect name and phone.
I would like to embed this form into an existing page that already exists.

Upon filling in this form, I would like to automatically create a contact in ClickSend.com.
This might need to be done via API.

Here is an example of an existing landing page:

As you can see there is a form that asks for name and phone.
Currently this form does not integrate with ClickSend.

I want to be able to embed an HTML form in the white box here.
